Mine Fatality Investigation Complete

The Occupational Safety and Health Service has completed its investigation into the death of Douglas Takerata Pourau.
Mr Pourau was killed when the truck he was driving fell 50 metres at the Martha Mine at Waihi on 26 June 2000.
The investigation undertaken by OSHs Hamilton Branch included exhaustive scrutiny of the work environment, the state of machinery and staff training, competence and fitness issues.
The investigation found no defects, legal breaches or critical findings against the mine company, contractors, employees or the victim during what was an extensive and detailed process.
OSH does not intend to release any further information regarding this tragic accident until the Coroner has had the chance to examine the incident.
